How is this different from a mental health survey?

A mental health survey focuses on one dimension. This wellness survey covers six: physical, mental, nutritional, social, financial, and environmental. It gives HR a complete picture of workforce wellbeing.

How many questions are in this survey?

36 questions. Six dimensions with 6 questions each. Takes about 10 minutes. Short enough for high response rates, comprehensive enough for actionable data.

How often should we run this survey?

Quarterly is ideal. The template includes a comparison scorecard for tracking trends. Annual is too infrequent. Monthly is too frequent for a comprehensive assessment.

Is this survey anonymous?

Yes. The core 36 questions collect no personally identifiable information. The optional demographics section asks only for department, tenure range, and work arrangement.

What do we do after collecting responses?
